Discharge care

Our hospital aftercare programme reaches all patients once they leave hospital.

More of our outpatients are now keeping appointments with specialists because we have already put a plan in place to help them. This means more patients will receive surgery or specialty advice to help them back to health.

We believe this service significantly reduces the rate of re-admissions to hospital.
